If you forgot you password, please type the following information:

  • PHC Member ID: Your Member ID number that is on your PHC Member ID card. If you do not have your PHC Member ID card, you can use your Social Security Number.
  • Username: Your Username that you used when you registered for your account.
  • Email address: Your email address that you used when you registered for your account.
